Confidentiality

All patient information is private. This includes anything in your medical record, and anything you give us. It also includes anything you tell your GOBHI Provider and clinical staff. We do not share this information without your approval except in an emergency or when required by state and federal regulations. In an emergency, only the information needed to help you is shared. State or federal staff may review your records to see if we gave you the best possible care.  Your GOBHI Provider may ask you to sign a release of information on your first visit or later. The form will tell what information is to be shared. It also will state who will receive the information and why they need it. The form also will list a date when the information sharing stops.
