Appointments

To make an appointment with your local GOBHI Provider refer to the list on pages 13-15. The GOBHI Provider will make your first appointment within two weeks of your call. Other appointments will be made after your first visit based on need.

If you cannot keep your appointment, let your GOBHI Provider know one or more days in advance. If you miss an appointment and forget to call, call as soon as you are able. Your GOBHI Provider will make another appointment for you.

If you miss many appointments, your GOBHI Provider may ask why this is happening. A GOBHI Provider will work with you to find out what can be done to meet your need for services. The GOBHI Provider may agree that services are no longer needed
